General Summary
St. Francis Urban Center is a small non-profit that strives to empower individuals to break the cycle of homelessness and address structural injustices to create institutional change. We are currently seeking a Family Case Manager to work in a supportive housing program for single mothers and their children. This position is imperative to the development and fulfillment of our mission and would be best filled by someone who is self-motivated, flexible, and creative.
Job Description
Family Case Manager
-assess needs of client and develop client goals
-assist client in staying focused on and achieving goals
-plan and conduct weekly visits with clients
-collaborate with client’s other case managers for a continuum of care
-provide emotional support to clients
-respond to crisis situations in an effective and efficient manner
-create individual service plans
-serve as a resource for budgeting and parenting education for mothers
-maintain documentation of achievement of goals and services provided
-creatively problem-solve issues that arise with clients
-create programming to meet the needs of clients
-use technology to disseminate agency information and updates
-develop and implement programming that utilizes available technological resources
Experience/Qualifications:
Bachelor Degree in human or social services mandatory, MSW/LCSW preferred
current Pennsylvania driver’s license with own transportation
two years’ experience working with children and families
vast knowledge of family structure and systems
experience in researching and writing grants
knowledge of available resources for the homeless population
proficiency in word processing, internet navigation, and ability and willingness to learn Adobe Contribute strongly preferred
